I saw this on Reba, when Chyanne had a baby shower for her daughter, Reba cut up and sewn together all of chyannes baby clothes.
So if you know how to sew, and your parent/guardian has you sisters old baby clothes, make the baby a quilt. It's cute.
OR.
Make a video tape(once again on reba), where everyone in the house talks to the baby through the camera, before the baby's born.
Just some suggestions

that is a very nice idea. maybe you could get a copy of the ultra sound pictures and put them on the cover and some pictures of mom and dad in there so almost like a babys first photo album of everyone in the family and maybe have everyone write soemthing nice in there.

That's a very sweet idea! And it's something your sister can treasure forever, seeing as it's completely personal.
Since the baby is coming around Christmas, maybe you could do a gift theme. Everybody knows children are a gift – and more so on Christmas! If you can, try to obtain some sonogram pictures from your sister of the little one and use them in the beginning of the book. Scrapbooking paper and supplies are in enless supply these days at just about every major retailer you could think of, or you could make your own. It would be really cute to cut out box shapes, attach them on three sides to a page, and slide in a reproduction of the sonogram photo. Then you could design a "lid" that would cover the top that could be pulled up in some way, to expose the little "gift" your family is about to receive. Just a thought for a starting point. You could either add to the book after the baby is born, or design lots of pages with room for photos to be inserted later.
When you're scrapbooking, don't limit yourself! You can use anything in a scrap book…fabric and ribbon, buttons, any small and relatively flat item will work. You can also feature other things than photos…keepsakes and momentos that aren't bulky are nice things to show off as well. Materials that are made specifically for scrapbooking are acid-free and won't damage your photos over time – these include papers, all those adorable embellishments that are available, and pens and markers suitable for drawing or writing. Generally these things can be pricey, but you can find deals if you shop around. And you don't have to use them – you can use other things, too. There are even software kits you can purchase to print your own papers and embellishments. If you choose to use less conventional elements, or to make your own paper (also a great possibility), get copies made of the photos you're going to use. That way you won't jeopardize the originals.
Whenever I wanted to scrapbook something I always found it helpful to go to a craft or hobby store where you can find examples of things the employees have done. There are endless websites you can look to for inspiration as well, and lots of craft stores offer classes in scrapbooking if you're really interested.
Good luck and congrats on your niece!

I also did this for a friend. I went to Walmart and bought some baby scrapbooking supplies (stickers, quotes, etc.) What I did was an 8 x 8 book (1 picture per page was all it needed) and included all the special holidays. For example first Halloween, first Thanksgiving, first 4th of July. I just put a colored paper (a little bigger than 4 x 6) and decorated around it. The colored paper is where your sister can put the pictures. For example, I put a red colored paper down for one page and put flags, stars, fireworks stickers around it and titled it "____'s first Fourth of July." Then for the other pages, you can put down the colored paper and just do a generic girl stickers…
Hope this makes sense…
